ENROLLMENT APPLICATION
600 Hour Nine Month Massage Training Program
Successful completion of this course is designed to prepare students for the (MBLEx) Massage and Bodywork Licensure Examination, and fulfill the State of Utah's educational requirements for Licensure as a Massage Therapist
The curriculum for Soma School of Massage, has been reviewed and approved by DOPL the Division of Occupational and Professional Licensing for the State of Utah and is registered under the Utah Postsecondary Proprietary School Act (Title 13 Chapter 34, Utah Code). Registration under the Utah Postsecondary Proprietary School Act does not mean the State of Utah supervises, recommends, nor accredits the school. It is the students responsibility to determine whether credits, or certificates will transfer to other institutions or meet employers' educational requirements.
Soma School of Massage is not accredited by a regional or national accreditation agency recognized by the United States Department of Education

Admission Qualifications

Admission Requirements:
- High school diploma, G.E.D., or Standardized Ability to Benefit Test (ABT) results
- Minimum age of 18 years
- Current photo I.D. (e.g., driver's license)
- Completed enrollment agreement
- $75.00 registration fee
Admission Procedures:
- Submit completed application form
- Pay registration fee
- Provide proof of education (high school diploma, G.E.D., ABT, or post-secondary transcript)
- Upon acceptance, submit photo ID
Registration Fees:
A $75.00 registration fee is required upon registration. This fee is refundable if cancelled within three business days. After three days but before the program start date, all monies will be returned minus the registration fee.
